Case managers coordinate all of the medical services required by a patient, including planning treatment, referrals, and follow-up so that care is continuous and comprehensive to maintain optimal health.

Spectrum has three full time case managers on staff with specialists for behavioral health, in-reach/outreach, men’s health, women’s health, and prenatal care.

Spectrum offers the following social services help:

  • Providing medical supplies or equipment you may need such as a glucometer
  • Assistance for home care and skilled nursing
  • Prescription assistance for the uninsured or under-insured
  • Referrals for additional services including behavioral health, food banks, substance use rehabs, and other community based organizations
