A Spectrum Health Services, Inc. patient may also report any patient safety concerns to The Joint Commission Quality and Patient Safety Office. To report a patient safety event or concern online, submit a new patient safety event or concern or submit an update to your incident (You must have your incident number). You may also submit the event or concern via mail, telephone, fax or email.
